하악과두의 인위적 병소에 대한 악관절 촬영법의 판독능 비교연구

Abstract

본 연구는 인공 하악과두상에 침식, 골증식체, 편평화등 인위적인 골 병소를 형성하여 파
노라마, 경두개, 경안와 촬영법, 측방 및 전후방 개별화 단층촬영법을 시행하여 하악과두 병
소에 대한 각 촬영법의 판독능을 비교하기 위하여 시행되었다.
결과는 다음과 같다.
1. 침식 병소의 판독능은 전후방 단층촬영법(96%), 측방 단층촬영법 (78%), 경안와
(59%), 경두개(56%), 파노라마 촬영법(48%) 순이었다.
2. 침식 병소의 위치별 판독능 비교에서 파노라마 촬영법은 내측 1/3부위, 경두개촬영 법
은 외측 1/3부위, 경안와 촬영법 은 전후방의 중앙부위, 측방 단층촬영법은 내외측의 중앙부
위 및 후방 1/3부위 에서 가장 우수한 판독능을 보였으며, 전후방 단층촬영법에서는 전외측
의 한 병소를 제외한 모든 병소를 확인할 수 있었다.
3. 골증식체의 관독능은 측방 단층촬영법 (100%), 경두개 촬영법(78%), 파노라마 촬영법
(56%) 순이었다.
4. 편평화의 판독에서 측방 단층촬영법에서는 하악과두의 형태 변화가 관찰되었으나, 파노
라마 및 경두개 촬영법에서는 전방부의 골 밀도저하만이 관찰되었다.
#초록#
The purpose of this study was to evaluate the detectability of various radiographic
techniques for mandibular condylar lesions. Erosive lesion, osteophyte and flattening
were formed on the artificial mandibular condyle, and panoramic, transcranial,
transorbital radiography, lateral and frontal topography were taken.
The results were as follows;
1. The detectability for erosive lesions was superior in the order of frontal
tomography(96%), lateral tomography(78%), transorbital(59%), transcranial (56%) and
panoramic(48%) radiography.
2. The location of erosive lesion that showed the highest detectability was the medial
third in panoramic, the lateral third in transcranial, the central portion of anteroposterior
direction in transorbital, the central portion of mediolateral direction and the posterior
third in lateral nomography Frontal tomography disclosed all erosive lesions except one
anterolateral lesion.
3. The detectability of osteophyte was 100% in lateral tomography, 78% in transcranial
and 56% in panoramic radiography.
4. For flattening, lateral tomography showed the flattened condyle, but both panoramic
and transcranial views showed only decreased bone density without the change of
condylar shape.
